Your membership and memorial brick can be obtained by filling out a brick paver order form and membership order form which are accessable using the buttons located below. You will need to print these forms and mail them to the Friends. Please use your best printing skills filling out the paver inscription, your phone number and email address, as we will want to contact you once the paver is installed.
A graphic character on the paver is included in this offer so you can ignore the $6 graphic charge shown on the order form, if you desire a graphic.
